Objective:The efficacy of a new oral formulation of finasteride (Proscar) in preventing prostate cancer has been evaluated in a double-blind, placebo-controlled clinical study.
Methods:A total of 998 patients with prostate cancer were enrolled in the study. The primary outcome measure was the incidence of prostate-specific antigen (PSA) rise and decrease in PSA, and the secondary outcome measure was the change in PSA and PSA-free prostate volume. In the placebo-controlled, double-blind, single-dose study, patients were treated with Proscar (0.3 mg/day) or placebo (1.0 mg/day). In the double-dose study, the patients were given finasteride (5 mg/day) for the first week to reduce the risk of prostate cancer. In the single-dose study, the patients were given Proscar for a further 7 days. During the study, a total of 546 patients were treated with Proscar. The overall incidence of prostate cancer was 12.8% (95% confidence interval [CI], 9.4–21.0) and the incidence of prostate-specific antigen (PSA) rise and decrease was 1.8% (95% CI, 0.2–6.8) in the Proscar-treated and placebo-treated groups, respectively. The incidence of PSA decrease was 0.6% (95% CI, 0.1–1.0) in the Proscar-treated group and 0.6% (95% CI, 0.1–1.0) in the placebo-treated group. The mean incidence of prostate-specific antigen (PSA) increase was 0.4% (95% CI, 0.1–1.0) in the Proscar-treated group and 0.6% (95% CI, 0.1–1.0) in the placebo-treated group. The PSA rise was 2.1% (95% CI, 0.7–4.2) and 3.4% (95% CI, 0.5–11.9) in the Proscar-treated group and 2.9% (95% CI, 0.7–9.2) and 8.4% (95% CI, 0.7–33.0) in the placebo-treated group. The PSA decrease was 3.8% (95% CI, 1.7–11.5) and 6.3% (95% CI, 0.8–23.0) in the Proscar-treated group and 5.4% (95% CI, 0.2–21.4) and 13.6% (95% CI, 0.7–41.3) in the placebo-treated group. The PSA increase was 2.8% (95% CI, 0.7–8.9) and 6.3% (95% CI, 0.8–23.0) in the Proscar-treated group and 5.3% (95% CI, 0.2–21.4) and 13.6% (95% CI, 0.7–41.3) in the placebo-treated group. Patients with an increase in prostate volume greater than or equal to 2% or a decrease in PSA was defined as an event.
Discussion:In the Proscar-treated group, the incidence of prostate-specific antigen rise and decrease was higher than the placebo-treated group. This suggests that finasteride (5 mg/day) may be useful to reduce the risk of prostate cancer in men with a normal prostate volume. The data are consistent with our finding that Proscar may have a protective effect in reducing the incidence of prostate cancer.
Conclusions:Proscar (5 mg/day) was a highly effective treatment for the prevention of prostate cancer. It may have a useful role in the treatment of prostate cancer.
